Because it is the insured who swears to the reality in the statements over the evidence of loss, the adjuster shouldn't try to dictate the contents of the form. The adjuster can help the insured but can not execute a sworn evidence of loss for him or her since the insured by yourself has the non-public knowledge that the statements in it are true.
